R Clay, really appreciate you sharing your stories and insights with us. The world would have so much more understanding and empathy if we all were a bit more open about our stories and how they have helped shaped our journey and worldview. Let’s jump in with a fun one: What makes you lose track of time—and find yourself again?
Luckily, I have a few….the two that have provided constant intermissions from the day to day stresses, the routine but nevertheless tough responsibilities of adult life and marriage, and the unexpected jolts from both being a urologic surgeon and having adult children.

I am no different than any other human I suppose. I do enjoy my vacations or as our friends across the pond say, “holidays.” But for me, finding myself alongside my wife on a beach and ocean, is the absolute bliss. Ambergris Caye, Turks and Caicos, Cap Cana (Dominican Republic), Anguilla, Hawaii, Cabo San Lucas, or Harbour Island — after 1-2 days, I completely decompress. Of course, everyone will read this and utter, “really…duh!! But my wife can confess, I literally change. But I also find a similar comfort and almost ethereal zen on our Chaparral 23 foot boat cruising Lake Tyler: listening to music, swimming and cruising around for hours. I can go to the lake with my wife, take friends in our boat or tie up to other friend’s boats, or even chill by myself.

But, traveling with my wife with or without our kids to Spain, Portugal, Italy, France, England…wherever, brings an escape. And on those trips, I am not relaxing and going slow!! Ha ha! No, I am dragging us on as many excursions and private tours as I can find and then adding more on my own. At the end of the day, everyone’s feet and ankles are destroyed. And my kids say I take enough photos and videos to make a documentary. And I dive deep into the lore and history of each vista, statue, castle, museum, or venue.

And lastly, our apartment. I am employed with the University of Texas Health East Texas as a board certified urologist in Tyler, Texas and love male and female reconstructive surgery/sexual medicine. But my wife and I have a company called Elite Enhancement of East Texas with an office in Dallas that we see clients on Fridays and Saturdays. Hotels are cumbersome and tiresome, so we leased an apartment in the Knox Henderson area a year ago, and even now, just walking into the apartment, is almost like walking into a resort lobby in Maui or Kauai for the first day of the trip…not joking…it has that escape feeling—it’s just my wife and I — no broken stuff at the house in Tyler, no honey-do’s, no bills and computer work, no discussing about the job…it’s a secret, fun, high rise getaway — and enjoy all the big city life has to offer — basically, it’s a 3-4 day weekend “holiday…” as our friends across the pond would say…….

Can you briefly introduce yourself and share what makes you or your brand unique?
I may have touched on this somewhat already.

I am a board certified urologist in Tyler, Texas, practicing now for 10 years. I graduated valedictorian from the University of North Texas Health Science Center, Texas College of Osteopathic Medicine…with no intention of doing so. In fact, I recall with intimate detail, telling my new wife, Michelle, whom I dated in college at Stephen F. Austin State University for 3 years, and calling my parents after 3 long days, that I was surrounded by the smartest, brightest, and impressive men and women I had ever met. If I could make all “B’s” in medical school, let alone survive the first semester and perhaps, with a touch of grace from our Heavenly Father and Lord Christ, I might…just maybe…actually graduate after the next four years. I let go of my college years, my party and fraternity attitude, and created a daily mantra: put your head down with your nose to the grindstone, no excuses, study hard, help your classmates, be a best friend to others, never forget your Lord and your wife, and do the best you can—and never lose yourself in the midst of this chaos. And I found myself valedictorian not only of my class at graduation, but was ranked the number one student all 8 semesters!

I do not boast this, I do not brag or even discuss it. My children just now realize it. Most of my friends do not know and probably would not care. Medical school is brutal. At least in my time, there were no trophies for “participation.” Medical school is not for the faint hearted—it is a beat down and you have to want it. And then, you have to apply for residency training in the specialty that you will perform the rest of your life. But what my classmates chose to do did not reflect necessarily their GPA or confidence. Oh sure, not all of us spent 2 years in general surgery residency followed by 4 years in urological surgery like me…some of my closest comrades spent 3 years in family practice or internal medicine residencies, both require 3 years less training. But those guys and girls, were top notch—brilliant. They wanted to commit themselves to society in a different manner, but in no way, less quality or intelligence.

So after completing 6 additional years of training beyond the 4 years of medical school, I found myself in Tyler, Texas. And over these 20 years, I have developed a niche in both male and female sexual medicine, pelvic floor disorders, and reconstructive surgery.. 99% of men and women who discover their interest in such a field, choose male or female..never both. To me, it only makes sense. I now have a trademark on a procedure called the “Minimally Invasive Single Incision Sling” or the “MISIS” for women with stress urinary incontinence and have been awarded a “Center of Excellence” in regards to penile prosthetic surgical implants with expertise in counseling patients as well as instructing medical students, residents, other urologists and NPs/PAs in erectile dysfunction/sexual medicine. My plan is to continue down this path, to help solidify UT as a quality center of excellence for patient outcomes and satisfaction in male reconstructive surgery and to propel UT into a similar level of quality of assurance and surgical expertise in female sexual medicine with pelvic floor and vaginal reconstruction.

I have already briefly mentioned about my other job, with my wife, Michelle—a company we started for men’s health called Elite Enhancement of East Texas. More of that to follow here shortly……

Amazing, so let’s take a moment to go back in time. What’s a moment that really shaped how you see the world?
I could answer this question in so many ways that it’s painfully difficult to choose only one as to not dilute the simplicity of the question, yet the depth and power of it…..

The world is quite bright and hopeful, yet simultaneously, dark and dismal. Life is complicated. And even in those moments, for even a mere split second, everything is in perfect harmony and celestial motion, there is chaos around the corner or perhaps, present, but we are blinded by the sheer bliss and comfort of normality and quiet…ssshhhhh. Because even as we gaze upon the heavens and see the beauty and tranquil moon and stars, the manner in which they exist depends upon a power and complicated universal tug of war that is mind blowing. Yet, we take those few seconds to gaze and enjoy the sky and to avoid asking the intricacies and to not question the “how” or “why” or “when.” Just enjoy the moment for what it is…regardless.

I recall interviewing for general surgery residency (long before I would change my mind, go back through the match, and flip over to urological surgery training — this added another year on top of the 5 year commitment). I was in New Orleans with my wife, Michelle, interviewing among others candidates who were out of my league: Boston College, Brigham, Columbia, UT, Vanderbilt, Brown, Harvard, SMU, Virginia, Duke, etc. We had never been to “Nu-ora-lins,” so we took it upon ourselves to do what any tourist would do…go to Pat O’Brien’s and seek out the tasty libations — namely, the infamous “Hurricanes.” Needless to say, not the greatest idea I have ever had the day before a rigorous interview process.

I remember having a typical hangover with upset stomach, headache, cotton mouth, fatigue, yada yada yada. I believe we each had 2-3 interviews with a professor or clinical doctor (as was the norm at each institution), then we found ourselves before lunch in a large room with all the chairs arranged in a horseshoe—with a single chair out front. We were asked to choose a chair and sit.
All around me were these peeps with name tags with their prestigious schools emblazoned. And I felt awful—I needed a Coke and some carbs fast…and Motrin. The chairman of the general surgery department took the single center chair and acknowledged that we had interviewed all morning long, and he would be reviewing the remarks. But he wanted each of us to say one thing about ourselves that we had NOT written down in our essays or applications or even in our interviews. On the spot, tell me something I don’t know or can’t find out about you. I was in the middle of the horseshoe and it started to my right. I desperately tried to muster up something…Eagle Scout at 14, EMT, psychiatric tech, fraternity, canoed Canada for 2 weeks, brown belt in karate, the number one student every semester on my way to being crowned valedictorian…nothing…I couldn’t come up with anything new — he already knew all of this. But these peeps were popping out piano and violin concertos, volunteering in Mercy ships; studied under this professor or that one; near death experience….I had nothing. And this wasn’t an option, this was for real…it was almost my turn….please dear Lord, hungover and limp, I need something honest and simple.

My turn. Now I cannot recall the exact working, but I promise you, this is not far from it because it was honest and simple.
“Sir, I appreciate having the opportunity to be here and interview. But my wife, who came with me on this trip, and I, have never been to Nu-oar-lins” before and we went to Pat O’Brien’s last night and had a few too many Hurricanes and had a blast. Great time!! We love your city!! But I am pretty sure that right now, I am not only hung over but may actually be a little drunk also. If you have any water and Motrin, I would appreciate it.” No lie, that’s gotta be 90% accurate. Honest and Simple. In the midst of the Harvard and Vanderbilt graduates’ list of accolades, I vomited up that final statement to the chairman, my future potential boss.
That was how I chose to leave a lasting impression.

True story—when the horseshoe terror had concluded, the chairman walked over to me and asked me to follow him
We went to his office whereby he provided me some water and from his desk drawer, offered me ibuprofen (same as Motrin), and I took it down fast. In so many words, he stated that I impressed him immensely because I was blunt—didn’t make excuses. I owned it. I admitted my issue, made light of it, but didn’t pass blame anywhere. Of all the answers, he enjoyed mine the most. He said he could work with a guy like me because he knew I would go the distance and would break my back to remain loyal and give him 100%. The program takes 4 residents a year…and that particular year, the 4 candidates they ranked in the top 4 slots, all accepted and committed. Months later I found out by the chairman himself that I had been ranked number 6 — haha! Me! A medical student from TCOM who received his undergraduate from little ole Stephen F Austin State University. Why me? What was it about me that the chairman found intriguing? Honest and Simple.

Be yourself because everyone else is already taken (not my quote). Sometimes, in those situations, they already know your paper credentials—they want to know can they trust you — can they rely upon you — are you brutally honest — will you bring the ship home during the stormy seas — can you put your nose to the grindstone and just do it — are you a team player?

What have been the defining wounds of your life—and how have you healed them?
Not sure how to reply to this question…..

I gain most of my wisdom from either things my dad has said or from the Bible. I love the book of James, one of Jesus’ brothers and of course, a disciple. Unlike Paul who wrote the majority of the New Testament and was quite authoritarian and ethereal, James was fairly blunt—some allegory like Paul, but most was just brutally honest and in your face. I love it in the first chapter where he states that when you face trouble (not “if,” but “when”), accept it as a part of life — sorry, but its facing you…embrace it, accept the challenge, pray about it, and tackle it head on with Jesus and allow yourself to grow and learn because of it — and in doing so, your strength will increase as will your endurance, But be careful, be mindful—-you must believe that you have permission to pray to our God — He will listen to you!! And when you do, watch for His answer or direction and believe it when He presents it to you, or your answer will be tossed and turned with no direction like a lost wave on the ocean.

Alright, so if you are open to it, let’s explore some philosophical questions that touch on your values and worldview. What do you believe is true but cannot prove?
How Heavenly Father and His son, Christ Jesus

well, one can prove the existence just by reading Proverbs and Psalms alone.
The creation of the Heavens, the solar system, the universe (and now, the theory that there is not only one universe but new proof which defies any known mathematical equation or explanation, that there a other universes beyond ours). Whatever happened millions of years ago, you cannot have a massive explosion of nothing — there has to be matter to explode or your so-called Big Bang Theory (also known as, hey, I am the Alpha and the Omega, I am artist, the potter, the Creator…and I want to get this party started…Bam!! Or should I say, “Bang!”). Show me anywhere where an explosion can occur without preexisting mater. Therefore, something had to exist if even milliseconds before a gazillion bazillion megaton collision of matter. So….where was this matter and how did it come to be. Something cannot come from nothing. And the universe is expanding faster.

Jesus fulfilled so many Old Testament prophecies, that it would be like wining the Power Ball a few times (not sure how accurate I am on that). And there are countless manuscripts being found more and more now that back up the historical prevalence of the Bible — documentation from sources outside simply finding another copy of a New Testament book.

I cannot “prove” that Jesus Christ is Lord and Savior in a way that is concrete. You cannot prove to me that Napoleon existed either — other than historical documents — same as Christ

Our DNA complexity with hair and eye color, blood type, genes that code for insulin receptors or muscle production, and neurochemical and hormonal engineering of the brain, heart, digestive system, etc, cannot be a slow evolution. The evidence does not support that whatsoever. And emotions and feelings…these cannot be random mutations that evolved over several millenniums…

God is real. No doubt.

Okay, so before we go, let’s tackle one more area. Are you tap dancing to work? Have you been that level of excited at any point in your career? If so, please tell us about those days. 
My wife’s and my company for three years now, Elite Enhancement of East Texas, that we see clients on Fridays and some Saturdays, is by far one of the times that I am literally tap dancing the moment we arrive at our Dallas apartment, McKenzie, in the Knox Henderson area. Every Thursday evening when we open the apartment door, I feel so relaxed—so sublime. It’s like a vacation every single weekend. And then. On Friday, when we open our office doors for these men of all ages, 21-80, to provide botulism neurotoxin or exosome/stem cell shots for erectile dysfunction, or a novel technique that I came up with for premature ejaculation, or UroFill penile girth, I am so giddy and happy it’s insane. It’s so wonderful to make these men feel so young again; to restore youthfulness and confidence. And working with my wife is probably one of the greatest things I have ever done and am so blessed that God allowed this to occur. I cannot fathom not working with her. She is spectacular on running our business 7 days a week, then letting me know who we are seeing and why, where they’re from, etc, If you had asked me if I would be in a partnership with Michelle, we both would’ve said you were crazy! But we love it! She’s the best.

Well over a decade now, Dr Paul Perito (Miami), a board certified urologist, invented the UroFill dermal filler penile girth enhancement procedure—in fact, among all the charlatans and other copy cat versions, UroFill is the only patented dermal filler world wide. He and his associate, Dr Alfie Saurez-Sarmiento, hand pick board certified urologists with specialization in andrology/male sexual medicine/penile prosthetic surgery. Almost 3 years ago, they chose to train me; and shortly thereafter, perfected my skills while getting my wife up to speed along with the the intricacies of the business, advertising, marketing, supply ordering — all about consistency in order to maintain results and brand. We have been active UroFill providers with clients from all over the state of Texas as well as Alabama, Kentucky, Washington, California, Arkansas, Tennessee, Kansas and Arizona just to mention a few.

Several weeks ago, Michelle and I attended what we consider the most important urological societal meeting each year, the Sexual Medicine Society of North America (SMSNA) and happened to be in Dallas, Texas. Late spring, Dr “Alfie” called us and stated that both he and Paul thought we should host the first ever, UroFill “Master Class” instructional course in the USA at our office to correspond with the SMSNA meeting. We were flabbergasted! What a great honor! Already certified UroFill providers would be invited to attend the course: to learn advanced injection techniques, to review difficult cases inherited from non UroFill providers and other charlatans/aestheticians and copy cats, to share business models, to promote and increase the brand, and to shape the future of male cosmetics. And what a great Friday morning it was! And to top it off, Paul and Alfie presented us with a plaque to commemorate Elite Enhancement of East Texas as the very first UroFill “Center of Excellence” in the country! We were humbled and overjoyed and honored. And to top it off, they named me as a UroFill “Master Injector” and was given the light blue UroFill scrubs to represent it. And make no doubt about it, we had a photographer present with pictures of the event. And to grab a picture of Paul, Alfie and me in front of our office black and white picture: Roger Moore as James Bond. We thought he represented the UroFill brand: sophistication, excellence, class, charisma, sex appeal, confidence…manly.

You ask what excites me….what makes me tap dance? I love practicing urology at the University of Texas Health East Texas Tyler for male and female sexual medicine and pelvic floor reconstruction; but I find absolute bliss and satisfaction working alongside my wife, Michelle, with our company, that we built together and manage together, answering to no one else…surrounded by like minded urologists with a shared goal and drive who love male cosmetics…and being embraced and trusted by Paul and Alfie.
At 55 years old, I cannot fathom another place. I am happy beyond words!!!
